Have a look at Printorium Bookworks http:// 
printoriumbookworks.islandblue.com/

They have detailed instructions on their website for how to do the  
layout so that PDF files for each page can be sent electronically.  
They will print and bind a proof copy so that you can see the final  
product before ordering a whole lot of books. Reasonable prices too  
(for Canada). There are US printers who will do the job cheaper. I  
would google "on demand book printing" for a wide range of options.
