[BCMA] paperwork for sending items out on conservation/repair?

Good morning,

 

What type of paperwork are collections departments using when sending items
out for conservation repair? Something formal.something informal? Does
someone have a form they would be willing to share?

 

We are considering using an external company to repair some books in our
holdings that have considerable value and I want to ensure a paper trail
with clear stipulations regarding responsibility for the items if anything
was to happen to them while offsite. I could modify a loan form but was
unsure if I should be using a different type of paperwork.
