[BCMA] Donor acknowledge forms

Hello Wendy,

We here at HAMM just spent a year fine tuning a simple one page acquisition form for our museum and almost immediately after printing the revised form on Wednesday we had a chance to see how well it worked.  Like a charm.  The form is adapted from three other island museums here in the area so that our museum is pretty much on the same page as the others.

I can send you a PDF of the form we use if you email me back with your direct email address.  I can also send you how this new form and system now functions in our small, but rapidly growing museum.
