[BCMA] Donor acknowledge forms

Hello all
I manage a very new and small museum and archive in the Greek community of
Victoria. I am learning as I go and have sorted out accessioning but would
like some help with a form to acknowledge donors but also nail down
museum's ownership versus loan of items and the museum's "right" to make
decisions related to keeping, displaying, disposing and conserving items.
An option for the donor to request conditions seems a reasonable component.

Does anyone have a simple form that I could customize?
