[BCMA] Loan agreement

[Moderated BCMA subscriber listserv.]

Hi Wendy,

Attached are our loan forms we use at the Nikkei National Museum. We often
extend our loans (incoming and outgoing) and I'd like to know if others
create a new form or include space on the original loan form to accommodate
extensions. I welcome any feedback.
